If you have read The Daily 5 Fostering Literacy Independence in the Elementary Classroom and ever wished there was a copy of our “Check-in form” (page 94 in our book) for you to use…here it is!

When we have our children gathered around us and they are “checking in”, letting us know which literacy choice they are making, we use the following form. Each child’s name is written in the left column, just like a traditional grade book page. We run off about 10 of these sheets once the kids names are written in, then keep them on a clipboard so they are ready to go. By placing the following initials in the boxes by the kid’s names as they check-in, we are able to keep track of the choices each child makes: R= Read to Self, RS=Read to Someone, W=Work on Writing, WW=Work Work and L=Listen to reading.
